How do I apply for a Blue Badge?

Before applying, please check your eligibility and then either download the appropriate application form below, or apply online on the Directgov website.

If you are unable to access the downloadable version of the application form, call us on 0300 123 7040 and we can arrange to have a copy of the form sent to you. 

Alternatively, you can collect and return an application form from any of the Council offices listed below:

Application forms can also be collected from and returned to all local libraries except from Elton, Saughall and Willaston libraries.
