300-helpdressingCheshire West and Chester Council is committed to supporting individuals in need of social care, to live independently in their local communities for as long as possible.


There is a national eligibility threshold for social care which consists of three criteria, all of which must be met for your needs to be eligible. The eligibility threshold is based on:

  • Whether your needs are due to a physical or mental impairment or illness.
  • To what extent your needs affect your ability to achieve two or more outcome, such as maintaining personal hygiene and managing toilet need: and
  • Whether, and to what extent, this impacts on your wellbeing.

Getting help

The Council has a duty to assess anyone who appears to have needs that mean they require care and support.

If you need an assessment, you or a family member, doctor or district nurse can make a referral for an assessment by contacting the Cheshire West Community Access Team:
