Our vision for Adult Social Care
We are committed to enabling people to live a great life and to support our communities to thrive. Having the right kind of housing with support is an essential part of this and this prospectus sets out what housing is required in the borough to support our vision.
Community Led Support is our underpinning practice model for social work and commissioning and means that all of our work now focuses on having good conversations with people and building upon the strengths and assets which exist in their life and their community.
We know that you cannot have a good conversation about how people can continue to live a great life without that conversation also being about housing. Housing can be a true enabler to people’s health and happiness.
We place great emphasis on listening to the voice of lived experience and coproducing solutions which have people’s needs at the heart. Our commissioning approach involves people at all stages and we expect supported living options to be no different.
We are looking to create diversity in our market and fundamentally we are aiming to improve lives and to achieve value for money. Our Market Position Statement sets out in more detail the kind of market we are looking to create and how we are working with providers, as key partners, to deliver our vision.

Our Enabling Great Lives Adult Social Care Strategy is underpinned by the following pillars:
- Our Cheshire West Place Partnership is operating effectively across current and proposed areas of integration.
- Equal voice of partners is maintained indecision-making.
- We co-produce and co-design, listening to the voice of people with lived experience.4. Joint Strategies are in place across key areas of focus and are regularly reviewed.
- Home First (enabling people to be independent in the place they call home)is a priority of all partners.
- Commissioning Intentions support the strengthening of community-based provision.
- Information is accessible to the public, commissioners and partners to support sign-posting and access to community provision.
- Community-led Support is embedded as an approach across the council and partners.
- Communication and support is consistently provided to support and enable ‘good conversations’.
- Prevention and early-intervention are key areas of focus for Adult Social Care and partners.
